SUPPORTS FOR COMMUNITY LIVING

Supports for Community Living (SCL) means community-based Kentucky Medicaid Waiver Services for individuals with mental retardation/developmental disabilities which supports an individual to remain in or return to the community in the least restrictive environment as an alternative to institutional care. An individual who is eligible for Medicaid and who meets the criteria for intermediate care for mental retardation may submit a completed application for SCL services that will currently result in placement on a central waiting list at the Division of Mental Retardation in Frankfort, Kentucky. Support Coordination provides the initiation and ongoing monitoring of admission, assessment, eligibility processes, and utilizes person-centered planning approaches to develop and monitor an individual support plan.

Residential Supports (except for room and board) facilitate independence and promote integration into the community for individuals who need to reside in an alternative living arrangement other than his/her own home. A staffed residence home is provided by CRCCC and includes 24-hour supervision, training, and other supports. A family home (foster-type) caregiver provides 24-hour supervision and training within his/her own home.

Community Living Supports facilitate independence and promote integration for individuals residing in their own homes to assist, train, and support in laundry skills, meal preparation, self-care, daily living skills, leisure choices, personal hygiene, etc.

Community Habilitation is a community-based program which provides supports, training, and intervention in areas of job preparation, self-care, daily living skills, social skills, safety skills, communication, etc. which enables an individual to participate in and to successfully utilize community resources.

Respite Supports are available on an hourly and/or daily basis and can be provided either where the person lives or in another setting.

Additional Supports/Services that can be provided to an individual who has the need for such supports include behavior support, supported employment, psychological services, speech therapy, physical therapy, occupational therapy, wellness monitoring, and specialized medical equipment and supplies.
